Pre-OP Diagnosis: IT temporo occipital deep-seated tumor

After prep. & drap under G.A. & in supine position with LT shoulder elevated slightly, head turned to RT side , a question mark incision over LT temporal region made, scalp flap elevated, percranium and temporalis muscle incision and after placing 5 burr holes skull flap elevated dura incised in the lower portion if the craniotomy site, dural flap elevated and fixed to pericranium, at first we looked sub temporally and there was no extra axial mass but cortex color changed to yellowish in the inferior aspect of temporal lobe, a 5 cm cortical incision over inferior temporal gyrus made and at depth of about 1.5-2 cm from the cortex we reacked to a non homogenous, with different colors (yellow, brown and pink in some area) , it was removed by suction & cautery, at the superior part the temporal horn opened and at the inferior side tumor passed the tentorium, the supratentorial portion was grossly totally removed but the portion in the posterior fossa was not removed, because total resection with such intra axial tumor is not possible and the patient may be in danger of deve loping neuralgic deficit ), after homeostasis under B.P. about 130 mm Hg dura closed with 40 silk and the space filled with fluid, bone flop turned and pericranium closed with 20 P.D.S. , bone dust filled into the lower holes, scalp closed in a layer separately, suction drain applied beneath scalp.
